AUTHORIZATION TO MAKE ADVANCE PAYMENTS
Advance payments of $[Amount of advance payment] Dollars per month may be made on this instrument. Each advance payment shall be applied first to the payment of the accrued interest and second to the payment of the principal. To the extent that an advance payment is applied to the payment of principal, interest shall cease on that part of the principal.
This is a UCC form. It is intended to be an addendum to other forms.
